Home Health Licensed Practical Nurse (LPN) in Frankfort, Ohio

Interim Health Care is hiring for Friday, Saturday, and Sunday day shifts—8 to 10 hours per day for a private duty case. You can choose every weekend or every other if that works better for you.

• $25.00 - $30.00/hour
• 1:1 nurse-to-patient ratios for focused care
• Flexible assignments, schedule control, true work‑life balance

• Provide skilled in‑home nursing care to patients with chronic conditions, post‑surgical needs, or other health challenges.
• Take vitals, perform assessments, document care, and update the RN supervisor.
• Support patients in following their care plan and learning self‑care techniques.
• Educate family members on care routines, medications, and home safety.
• Work alongside a care team including RNs, CNAs, and physicians.
• Contribute to discharge planning and ongoing care coordination.

• Active Ohio LPN license.
• Graduate of an accredited LPN program.
• At least 1 year of LPN experience, ideally in home health.
• CPR certification.
• Familiarity with home health regulations (state and federal).
• Clinical confidence, strong communication, and a compassionate heart.
