We are currently seeking a dependable and compassionate Licensed Nurse (LPN or RN) to provide twice-daily skilled nursing visits for a client in Owensville, Ohio. This role involves providing morning and evening skilled nursing visits in the client’s home. Ideal for nurses seeking consistent, structured visits with minimal travel.

• Provide skilled nursing care per physician orders and plan of care
• Medication administration and management
• Health assessments and vital signs monitoring
• Documentation in accordance with OASIS and agency standards
• Communication with physicians, case managers, and family members
• Ensure client safety and quality of care

• Twice daily visits (AM & PM)
• Flexible timing within scheduled windows
• Consistent ongoing case

• Active Ohio LPN or RN license (required)
• Home health experience preferred
• Current CPR certification
• Reliable transportation and valid driver’s license
• Ability to chart accurately and timely
• Strong clinical judgment and communication skills
