Primary Care Nurse Practitioner

Defiance, Ohio – Salary: $100,000 - $110,000 per year

Schedule: Monday–Friday, 8:00 AM – 5:00 PM EST (4 days per week – flexible with management)

Position Overview

We are seeking a compassionate and experienced Nurse Practitioner (NP) to provide high‑quality primary and preventative care to residents in skilled nursing facilities in the Defiance, Ohio area. This role focuses on long‑term care, chronic disease management, and collaboration with interdisciplinary teams to ensure comprehensive, patient‑centered care.

Key Responsibilities
  • Provide primary and preventative medical care to residents in skilled nursing facilities
  • Assess and manage residents experiencing acute changes in condition
  • Perform advanced care planning and chronic care management
  • Prescribe medications, including controlled substances, in compliance with Ohio state law
  • Order, review, and follow up on diagnostic tests and treatments
  • Communicate regularly with nursing home staff regarding resident status and care plans
  • Collaborate with primary care physicians and interdisciplinary care teams
  • Maintain accurate and timely documentation within the electronic medical record (EMR)
Required Qualifications
  • Current, unrestricted Nurse Practitioner license in Ohio
  • Board Certification as a Nurse Practitioner
  • Active DEA license
  • Clinical background in Adult, Family, or Geriatric medicine
  • Prior experience working in a long‑term care or skilled nursing setting
  • Proficiency with electronic medical records (EMR systems)
  • Ability to manage patients with advanced illness and multiple chronic conditions
  • Strong collaboration skills within an interdisciplinary care team
  • Psychiatric experience is a plus
Preferred Qualifications
  • Experience with complex geriatric populations
  • Strong communication and clinical decision‑making skills
  • Commitment to patient‑centered, compassionate care
Schedule & Flexibility
  • Standard hours: 8:00 AM – 5:00 PM EST
  • 4‑day workweek (days determined collaboratively with management)
