Join us for in‑home clinical assessments providing comprehensive health examinations.

Key Responsibilities

• Conduct adult/geriatric assessments, including medical history, diagnosis, treatment, health education, physician referrals, case management, and documentation per Matrix guidelines.
• Administer point‑of‑care and lab testing as needed.
• Coordinate care with physicians, case managers, social workers, family members, and other medical personnel.
• Collaborate with primary care physicians on patient education and follow‑up.
• Provide services at members’ residences or skilled nursing facilities.

• Competitive pay – $1,800–$11,500 per month based on average visits, with rates $90–$115 per assessment.

Qualifications

• Master’s degree or equivalent experience and NP licensure.
• Current NP licensure with prescription authority (or ability to obtain).
• AANP, AACN, or ANCC board certification in an approved specialty.
• Current BLS, ACLS, or CPR certification.
• Preferred one year NP experience; new grads encouraged to apply.
• Valid state driver’s license and proof of adequate automobile insurance.
• Strong computer skills and familiarity with electronic health record systems.
• Excellent verbal and written communication skills.
