Job Summary
The Social Worker provides comprehensive psychosocial assessment, counseling, care coordination, and advocacy services to individuals and families. This role supports clients in navigating social, emotional, behavioral, and environmental challenges while promoting overall well-being and access to appropriate resources. The Social Worker collaborates with interdisciplinary teams to develop and implement effective care plans in compliance with professional, ethical, and regulatory standards.
Key Responsibilities
Conduct psychosocial assessments and develop individualized service or treatment plans
Provide individual, group, and/or family counseling within scope of licensure
Assist clients with coping strategies, crisis intervention, and problem-solving skills
Coordinate care and connect clients with community resources, social services, and support systems
Advocate for clients to ensure access to appropriate services and continuity of care
Maintain accurate, timely, and confidential documentation in accordance with agency, state, and federal requirements
Collaborate with healthcare providers, case managers, and community agencies
Participate in interdisciplinary team meetings, trainings, and policy reviews
Monitor client progress and modify service plans as needed
Ensure compliance with ethical standards and applicable laws and regulations
Qualifications
Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited program
Active social work license or eligibility for licensure (LSW, LPC equivalent, depending on role)
Knowledge of social service systems, behavioral health, and community resources
Strong communication, organizational, and interpersonal skills
Ability to manage complex cases and work with diverse populations
