Reentry Specialist Job Description
DAILY DUTIES:
- Reports to the Reentry Coordinator
- Coordinate/communicate with all other departments
- Review daily the RA Log, Shift Reports, and PM Meeting Reports
- Check email to ensure up to the hour communications
WEEKLY DUTIES:
- Participate in reentry and house meetings
- Manage the case of each resident on caseload for the entirety of their program
- Maintain contact with probation departments in terms of resident progress
- Make timely and appropriate case notes for residents on caseload
- Ensure resident case records are up to date
- Utilize both concern slips and the STAR reward system in addressing resident behaviors.
- Distribute Reentry Needs Reports to new intakes and submit to Reentry Coordinator
- Send Mail/Phone/Visit Forms to residents’ Probation Officers for approval
- Complete PREA reviews
- Create and distribute Reentry Needs Plans for all residents on caseload
- Facilitate reentry groups and classes
- Set up reentry appointments
- Approve visitation requests for residents on caseload
- Conduct Family Outreach
- Participate in Loved Ones Group
- Facilitate the program pass process when necessary
- Recommend and fill out Advocacy Reviews for residents
- Transport residents to miscellaneous appointments when needed
- Recommend residents to be discharged from the program based on behaviors
- Complete Continuing Care Plans for residents on caseload and ensure they are sent to the corresponding probation officers in a timely fashion
- Close out resident files and case records in the schedule amount of time
EXTRANEOUS DUTIES:
- Suggest ways to make programming better for the resident
- Represent STAR in court when necessary
- Complete any other jobs assigned to by management
EDUCATION REQUIREMENTS:
- Bachelor’s Degree or better in human service or criminal justice field is preferred.
- Required to earn, at minimum, the CDCA licensure
MINIMUM QUALIFICATIONS:
The applicant must show proof of holding and must continue to maintain valid permits, licenses and certifications as may be required for this, or demonstrably similar, position as recognized by the Ohio Department of Administrative Services or as otherwise specified by the Ohio Revised Code or applicable Administrative Codes.
The applicant must have a valid Driver’s License. Applicant must have the ability to deal with and solve problems involving several variables in somewhat unfamiliar settings and topics. Applicant must convey a strong interest in being involved in offender rehabilitation and specific concepts of Community Justice.
