Join Flagship Therapy as a full‑time Physical Therapist at our Mt. Ogden Health & Rehabilitation facility, a post‑acute care center offering long‑term, short‑term, and geriatric services. • Conduct screenings and evaluations of patients with physical functioning disorders and/or wound care, developing appropriate care plans that adhere to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients, following regulatory and clinical practice requirements.
• Administer a variety of procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices, with wound care as needed.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

• Physical Therapy license required.
