Position: Licensed Vocational Nurse Med Surg FT Days

Patient Care

  • Perform comprehensive head‑to‑toe assessments and reassessments under the direct supervision of a Registered Nurse, in accordance with policy, for adolescent, geriatric, and general patient populations.
  • Assess and reassess pain effectively; apply appropriate pain management techniques and educate patients and families accordingly.
  • Deliver patient care that aligns with age‑specific standards and individual needs.
  • Demonstrate knowledge of medications and administer them accurately based on patient age and clinical condition.
  • Follow the five rights of medication administration to minimize the risk of errors.
  • Develop and implement teaching plans based on identified patient learning needs; evaluate effectiveness and include family education when appropriate.
  • Assist physicians with procedures and perform technical and manual nursing tasks under RN supervision.
  • Provide treatments and services within the scope of LVN/LPN licensure.
  • Treat patients and families with respect and dignity, addressing psychosocial, cultural, ethnic, and spiritual needs.
  • Engage patients and families in care planning and maintain professional communication.
  • Identify and communicate psychosocial needs to Social Services or Discharge Planners for both in‑hospital and post‑discharge support.
  • Apply knowledge of growth and development to deliver age‑appropriate care and interpret patient data effectively.
  • Perform all aspects of patient care in a safe environment that reduces the risk of medical and healthcare errors.
  • Maintain medical and surgical asepsis during treatments and procedures.
  • Coordinate and supervise patient care under the direct supervision of a Registered Nurse.
  • Collaborate with other departments to ensure an interdisciplinary approach to patient care.
  • Respond appropriately to patients in emergency or physically distressing situations.
  • Provide care appropriate to the condition and age of the patient, including adolescent, geriatric, and general populations.
  • Maintain patient confidentiality at all times.
  • Perform other duties as assigned within the LVN/LPN scope of practice.
  • Communicate clearly and appropriately with team leaders, coworkers, and physicians.
  • Foster positive working relationships within the department and across hospital units.
  • Maintain clinical competency in stroke care; complete ongoing education and required stroke‑related competencies.
  • Serve as a hospital‑wide resource for stroke care, regardless of the patient’s admitting diagnosis.
  • Manage and coordinate stroke patient care throughout hospitalization and post‑discharge.
  • Ensure documentation meets current standards and complies with hospital policies.
Safety Practices
  • Promotion of a culture of patient safety which resulted in the identification and reduction of unsafe practices.
  • Utilization of Culture of Safety survey responses in improvement efforts.
  • Identification of risks and hazards to patients through various risk assessments, with actions taken to mitigate patient harm.
  • Improvement in medication safety practices, including increased reporting of adverse drug events and improved compliance with the medication reconciliation process.
  • Decreased hospital‑acquired infections and improved compliance with infection control practices (including hand hygiene).
  • Decreased hospital‑acquired conditions such as complications associated with ventilator use and pressure ulcers, through improved compliance with patient safety bundles and/or other formal performance improvement programs.
  • Manages and operates equipment safely and correctly.
  • Demonstrates understanding of suicide prevention and patient restraints for sitting assignments of 1:1 care, frequent checks and continuous observation of patients.
Professional Responsibility
  • Maintain a neat and professional appearance in accordance with the organization’s dress code.
  • Complete all required annual education and training.
  • Ensure compliance with applicable regulatory requirements.
  • Report to work on time and as scheduled; complete assigned duties within designated time frames.
  • Wear proper identification while on duty and accurately utilize the computerized timekeeping system.
  • Complete required in‑services and submit documentation promptly.
  • Attend annual performance reviews and scheduled departmental in‑services.
  • Participate in at least 75% of staff meetings annually.
  • Represent the organization in a consistently positive and professional manner.
  • Actively engage in performance improvement initiatives.
  • Adhere to all organizational policies related to ethical business practices.
  • Promote and communicate the mission, values, and goals of the facility.
Education
  • Graduate of Vocational Nurse from an accredited Nursing Program
Experience
  • Minimum 2 years’ experience in Nursing units—Preferred
