Enhanced Care Management (ECM) Lead Care Manager - San Diego County

Pacific Health Group is seeking a dedicated Lead Care Manager to guide members through comprehensive care plans. This role focuses on addressing social determinants of health, coordinating services, building trusting relationships, and improving overall quality of life.

Base Pay

$25–$29 per hour

Work Details

Job type: Full-time
Hours: 40 per week (8‑hour shift, Monday–Friday, 8:30am–5:00pm PST)
Location: Hybrid remote in San Diego County, CA – on the road

Responsibilities
  • Conduct frequent in‑person visits to members at home or community centers, establishing trust and gathering first‑hand insights.
  • Coordinate all services (medical appointments, transportation, housing, financial support) to ensure continuity of care.
  • Act as a case manager with empathy: maintain close contact via phone, video, and in‑person meetings, monitor progress, and address new barriers.
  • Build relationships with community resources (housing assistance, job training, childcare) and link members to those services.
  • Advocate for members’ rights—secure timely treatments, insurance authorizations, and fair access to services.
  • Serve as the central point of contact among members, families, healthcare teams, and community organizations; facilitate care conferences.
  • Document assessments, care plans, and progress notes accurately and timely.
  • Use data and observations to refine care strategies and improve program effectiveness.
  • Maintain regulatory compliance with Medi‑Cal, CalAIM, and other relevant regulations.
  • Pursue professional development through trainings and workshops to enhance skills (cultural competence, motivational interviewing, crisis intervention).
    • Additional duties: remain flexible, share best practices, and support team needs.
Requirements
  • Residency:
    Must reside in San Diego County.
  • Experience:

    3–5 years in case management, social services, or healthcare.
  • Expertise:
    Familiarity with Medi‑Cal, CalAIM, and Enhanced Care Management.
  • Knowledge of healthcare systems and local community resources.
  • Strong communication, empathy, and cultural competence.
  • Time‑management skills and attention to detail.
  • Proficient with case‑management software and related tools.
  • Valid California Driver’s License (Class C minimum); personal, operable vehicle with liability insurance; MVR background check required.
  • Successful completion of a pre‑screen assessment.
