Work Setting:
Grant funded position in the Healthy Start Program, requiring travel and home visits.
- Provide independent professional Human Services Counseling and care coordination for the Healthy Start Program.
- Conduct home visits to assess high‑risk prenatal and infant clients, identify barriers, and develop Family Support Plans.
- Link clients with community services, counsel on prenatal care, nutrition, infant health, and safety.
- Document services and maintain records within 72 hours.
- Coordinate with Marion County Health Department clinics, private providers, and other agencies to meet clients’ medical and social needs.
- Implement group educational services for high‑risk clients as per program guidelines.
- Prepare and submit local, district, and state reports.
- Attend staff meetings, workshops, and training to maintain professional knowledge.
- Perform additional duties as assigned.
- Represent Healthy Start at community fairs and events, including occasional weekends.
- Case management experience and knowledge of Healthy Start.
- Understanding of assessments, interviewing, education theories, and techniques.
- Proficient data entry and computer‑based record keeping.
- Strong communication and interviewing skills.
- Ability to establish and maintain effective workplace relationships.
- Capability to provide group education.
- Organizational and planning skills.
- Self‑direction and independence.
- Fluent in Spanish for client communication (if applicable).
- Willingness to travel, work in home settings, and attend events on weekends.
- Valid driver’s license.
- Associate degree or equivalent experience providing medical or social services to women or children per Healthy Start Program Standards and Guidelines 2019.
- Willingness to work in the office and perform field duties.
- Experience in maternal and child health.
- Experience in case management.
- Experience working with community partners.
