Physical Therapist Assistant – Home Health

Center Well Home Health is a Humana subsidiary specializing in personalized home care, providing a full range of services including physical therapy. This role supports patients in the comfort of their homes, helping them regain strength, mobility, and independence.

• Administer the physical therapy treatment plan as developed by the Physical Therapist.
• Administer non‑complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities to patients.
• Accurately document daily, weekly or monthly records of clients and submit clinical paperwork on time.
• Attend educational meetings and coordinate and contribute to department in‑services.
• Notify the physical therapist or Executive Director of any equipment failures, lack of supplies or other changes.
• Schedule clients and perform varied office tasks.
• Apply infection control and universal precautions in patient care activities.
• Accept clinical assignments consistent with education and competence to care for patients.
• Promote compliance with payor requirements through education, coaching and assistance as necessary.

• Associate degree in Physical Therapy or related field.
• Current state license to practice Physical Therapy.
• Valid driver’s license with reliable transportation.
• Proof of current CPR certification.
• Minimum two years experience as a Physical Therapy Assistant.
• Knowledge of geriatric population changes.
• Self‑directed, enthusiastic and receptive to feedback.
• Good verbal and written communication and organization skills.
• Professional image and attitude toward patients, families, and coworkers.
• Meets continuing education requirements and maintains clinical competency.
• Flexible schedule and local travel ability.
• Maintains confidentiality.
• Fluency in English.
• Good attendance record.
• Performs other related duties as assigned.

• $58,400 – $80,000 per year base salary.
• $37.00 – $52.00 per visit/unit.

Center Well Home Health provides personalized comprehensive home care for patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization.
