Physical Therapist Assistant – Home Health
Center Well Home Health is a Humana subsidiary specializing in personalized home care, providing a full range of services including physical therapy. This role supports patients in the comfort of their homes, helping them regain strength, mobility, and independence.
Responsibilities- Administer the physical therapy treatment plan as developed by the Physical Therapist.
- Administer non‑complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities to patients.
- Accurately document daily, weekly or monthly records of clients and submit clinical paperwork on time.
- Attend educational meetings and coordinate and contribute to department in‑services.
- Notify the physical therapist or Executive Director of any equipment failures, lack of supplies or other changes.
- Schedule clients and perform varied office tasks.
- Apply infection control and universal precautions in patient care activities.
- Accept clinical assignments consistent with education and competence to care for patients.
- Promote compliance with payor requirements through education, coaching and assistance as necessary.
- Associate degree in Physical Therapy or related field.
- Current state license to practice Physical Therapy.
- Valid driver’s license with reliable transportation.
- Proof of current CPR certification.
- Minimum two years experience as a Physical Therapy Assistant.
- Knowledge of geriatric population changes.
- Self‑directed, enthusiastic and receptive to feedback.
- Good verbal and written communication and organization skills.
- Professional image and attitude toward patients, families, and coworkers.
- Meets continuing education requirements and maintains clinical competency.
- Flexible schedule and local travel ability.
- Maintains confidentiality.
- Fluency in English.
- Good attendance record.
- Performs other related duties as assigned.
- $58,400 – $80,000 per year base salary.
- $37.00 – $52.00 per visit/unit.
