  • Hybrid role – 75% travel within county

Position Purpose

Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost‑effective healthcare outcomes. May develop or assist with developing personalized service care plans for long‑term care members and educate members and their families/caregivers on available services and benefits.

Key Responsibilities
  • Evaluates member needs, available resources, and recommends or facilitates the best outcome plan.
  • Assists with developing ongoing long‑term care plans and identifies required providers, specialists, and community resources.
  • Coordinates between members/families and the care provider team to ensure services are accessible.
  • Provides resource support to members and families for various needs (e.g., employment, housing, participant direction, independent living, justice, foster care).
  • Monitors care plans, member status, and outcomes, and provides recommendations based on identified needs.
  • Interacts with long‑term care healthcare providers and partners to ensure member needs are met.
  • Collects, documents, and maintains member information and care activities to ensure compliance with state, federal, and payer regulations.
  • May perform home and/or other site visits to assess needs and collaborate with providers.
  • Provides and facilitates education on procedures, provider instructions, service options, referrals, and benefits.
  • Provides feedback to leadership on opportunities to improve quality of care and service delivery.
  • Performs other duties as assigned.
  • Complies with all policies and standards.
Education / Experience

Requires a Bachelor's degree and 2–4 years of related experience or equivalent experience through accomplishments of applicable knowledge, duties, scope, and skill reflective of this level.

Pay Range

$26.50–$47.59 per hour.
