Transitional Support Worker

Overview

Member of the interdisciplinary team the transitional Support worker is responsible for assisting youth in their successful integration into the community from a secure setting. This is accomplished through rehearsing and instructing youth in the practical application of the skills and coping strategies acquired during their stay  TSW completes pass and pass programs along with supervising youth in the community while they practice their skills.

TSW connects youth to community agencies and placements to prepare youth for discharge.

• Provide care for youth through the therapeutic milieu, attend plan of cares, prepare pass programs and pass for youth. Supervise youth within SAYC along with supervise youth on pass. Connect youth to community based programs and recreational/leisure passes. Prepare youth for discharge, attend placements with youth in assisting them integrate back into the community or less restrictive environment. Involved in working with the multi-disciplinary team in planning care for youth in a secure environment
• Prevent or manage aggressive behavior. Engage in case management duties for youth. Supervise youth within the unit or during visits. Attend Plan of cares, clinical meetings, team meetings. Community agency meetings
• Prepare written reports, incident reports, Daily Observations, communication book enteries, Movement logs. Contacting community agencies for information to support youth at SAYC
• Process Admissions and Discharges
• Record and administer approved medications, unit searches

• Requires completion of Community College Diploma) or equivalent
• Some practical or position related experience required
• Satisfactory Criminal Record Check (including a Vulnerable Sector Screening) and Pre
  - Employment Medical
• Bilingualism (English and French) considered an asset
