, you'll play a key role in supporting clients on their wellness journey - helping them navigate health and community care services, develop personalized care plans, and connect with the resources they need to thrive.

Acclaim Health announces the availability of one (1) Full Time Wellness Care Manager position (equivalent to 75 hours in a biweekly period). As a member of the Community Support team, the incumbent will be supporting clients throughout Halton Region. This will include a combination of in home visits, office visits and virtual visits. The Individual is required to work Monday to Friday from 8:30am – 5:00pm with the flexibility to work evenings as needed.

Pay Rate: $28.84 - $30.58 per hour

Duties & Responsibilities
  • Delivers a range of Wellness and Complementary Therapy services that are delivered in clients’ homes and in group settings.
  • Ensures Wellness services are aligned with best practice.
  • Coordinates and manages the introduction of new therapies and service offerings.
  • Completes client assessments to determine eligibility and to determine specific clients’ needs and appropriate services.
  • Works with the client to develop a care plan to meet needs identified.
  • Provides referrals to other internal supports and external community resources as needed.
  • Works in a manner that ensures client safety.
  • Provides support and appropriate training and/or arranges the training for Complementary Therapy volunteers
  • Provides supervision, training, mentoring and support of volunteers
  • Matches volunteers with clients to provide therapies and monitors clients’ well-being while receiving therapies.
  • Provides education sessions and workshop on Wellness topics
  • Updates and maintains confidential client and volunteer files in accordance to established policies and procedures.
  • Responds to all telephone calls and emails and inquiries with appropriate and timely interventions.
  • Gathers statistics for monthly and quarterly statistical reports as they relate to specific program.
  • Assists with the planning, coordinating and facilitating volunteer education sessions and recognition events
  • Promotes quality/risk management and provides related education to volunteers and students, ensuring that risk/and incident management policies and procedures are followed.
  • Represents Acclaim Health in the community promoting linkages to appropriate community resources as requested by the Director.
  • Ensures that workplace conduct and activities are in accordance with the current Occupational Health and Safety Act and internal health and safety systems.
  • Performs other related duties associated with the position and as requested by Director.
Qualifications & Education
  • Post-secondary education in health, social services or related discipline
  • Qualified to train individuals in at least one complementary therapy (Therapeutic Touch, Reiki)
  • Valid Ontario Driver’s License and insurance coverage in accordance with the company policy.
Experience & Skills
  • Minimum 2 to 3 years demonstrated experience working with Complementary Therapies
  • Understanding of and adherence to hospice philosophy of care
  • Understanding of dementia care
  • Experience working effectively with
  • Strong verbal, written communication and interpersonal
  • Knowledge and understanding of volunteerism, community resources and the Halton healthcare system
  • Effective decision-making, problem-solving and organizational skills.
  • Demonstrated knowledge in quality/risk
  • Proven ability to multi-task and meet
  • Proficient in Microsoft Office 365,
  • Demonstrated ability to work independently and collaboratively as a team member
