Job SummaryOur client is seeking a full‑time Behavioral Health Provider to deliver both traditional clinical therapy and services utilizing the Primary Care Behavioral Health (PCBH) model in an integrated outpatient medical setting. This role involves collaborating with primary care teams to assess, treat, and support patients through brief, evidence‑based therapeutic interventions. The ideal candidate is compassionate, adaptable, and thrives in a fast‑paced, team‑based healthcare environment while maintaining the highest standards of ethical and professional conduct.
Job Responsibilities- Partner with members of the interdisciplinary care team to provide behavioral health services within a PCBH model
- Assess patients presenting in a primary care setting and determine appropriate treatment plans and resources
- Utilize evidence‑based therapeutic techniques tailored to individual patient needs
- Conduct brief assessments and crisis interventions, including development of individualized safety response plans
- Provide individual and/or family therapy using brief, solution‑focused interventions
- Develop, document, evaluate, and adjust individualized treatment plans in compliance with COMAR regulations, including required six‑month reviews
- Facilitate seamless transitions of care between primary care providers and behavioral health services
- Complete clinical documentation within 24 hours in accordance with COMAR and HIPAA requirements
- Maintain a safe, ethical, and supportive environment that encourages patient trust and engagement
- Effectively manage multiple 16–30 minute sessions in a fast‑paced clinical setting
- Demonstrate empathy, compassion, and cultural sensitivity when working with diverse patient populations
- Support the ongoing development and growth of the behavioral health program
- Deliver excellent, patient‑centered customer service to patients and colleagues
- Uphold applicable professional codes of ethics and align with organizational core values, including patient focus, respect, accountability, teamwork, continuous improvement, and integrity
- Perform other duties as assigned
- Graduate degree in Social Work, Psychology, or a related field
- Licensure preferred:
Licensed Certified Social Worker – Clinical (LCSW-C) or Licensed Clinical Psychologist (PsyD) - Accepted:
Licensed Masters Social Worker (LMSW) or Licensed Graduate Social Worker (LGSW) actively working toward clinical licensure - Valid driver’s license required
- Strong communication, planning, organizational, and documentation skills
- Ability to multitask with strong attention to detail
- Knowledge of HIPAA regulations and healthcare documentation standards
- Ability to work collaboratively within a team‑based, patient‑centered model of care
- Demonstrated dependability, punctuality, and professionalism in a medical or clinical environment
- Sensitivity to and understanding of diverse cultural, social, and economic backgrounds
- Physical ability to work in an outpatient clinic setting, including sitting, standing, walking, lifting up to 15 pounds, and assisting patients with limited mobility
