Position SummaryProvide Case Management services to individuals with intellectual and/or developmental disabilities. Key objectives include establishing positive working relationships, exploring options, and developing growth opportunities for each person served. Establish and monitor a Person-Centered Plan (PCP) and ensure services meet state-mandated standards. Identify and secure ongoing support services and assist individuals with development and implementation of PCPs that document service needs and outcomes.
Responsibilities(Essential Duties)
- Advocate for individuals, provide resource information, and educate them to become more self-determined
- Educate individuals and their families about services and coordination
- Encourage empowerment and independence
- Collect information for eligibility for DDA services and recommend eligibility and priority
- Meet with individuals/families to gather information; obtain and document necessary information
- Complete Comprehensive Assessments (CA) and plan/update PCPs
- Develop and write PCPs in collaboration with the individual; conduct PCP preparations
- Research, identify, refer, and coordinate resources to achieve PCP outcomes; share resources with the team
- Travel to meet with support staff as needed
- Provide information, make referrals, assist individuals, and support transitions
- Complete required paperwork and follow up for waivers; identify services and coordinate with providers for transitions from state facilities or for those with forensic involvement
- Monitor PCPs and conduct timely visits/contacts per regulations
- Submit required documents (RECERT/RECON/AIR) and identify new medical/health services as needed
- Adhere to SCI policies and procedures; meet performance utilization benchmarks
- Maintain HIPAA records securely and utilize tracking/time management tools

- A bachelor's degree in a human services field from an accredited program; OR
- A bachelor's degree in a non-human services field from an accredited program with two years of experience in a human services field
