JOB SUMMARYUnder the supervision of the Social Work Supervisor, the Social Worker Assistant will provide social work services to participants of Hong Fook Centers. This includes intake of prospective participants, providing language assistance, working with other team members to provide care management services, and assisting the Social Work Supervisor in the psycho‑social assessment of participants.
MAJOR DUTIES AND RESPONSIBILITIES- Introduce Hong Fook Adult Day Health Care programs to prospective participants and their families.
- Perform intake, home visit, and initial assessment of potential participants for services.
- Follow up on the process to bring participants in for program assessment, including obtaining medical reports, scheduling transportation, and notifying team members.
- Enroll program participants, explain program rules and participant rights, and assist new participants' adjustment to the program.
- Provide care management services to the participants.
- Assist program participants to obtain needed services and benefits, and act as liaison with outside agencies.
- Interpret various services or benefit notices and medical insurance statements for program participants or family members.
- Assist the Social Work Supervisor in the periodic assessment of program participants, and maintain current written records in compliance with licensing requirements.
- Assist in or help conduct support groups for program participants.
- Participate in and help organize outreach activities to the target frail elderly and disabled adult population.
- Participate as a member of the multidisciplinary team.
- Perform other appropriate tasks deemed necessary by the supervisor.
- BA in Social Work or related field.
- Experienced and interested in working with frail elderly or the disabled population.
- Able to work in a diversified environment.
- Able to work independently and as a member of a multidisciplinary team.
- Good oral and written communication skills.
- Ability to make public presentation a plus.
- Bilingual in English and Cantonese/Mandarin desirable; ability to communicate in one of the targeted Asian languages (Vietnamese, Filipino, other Southeast Asian language, and Korean) also desirable.
- Flexible in work shifts to meet program needs.
- Possession of a valid CA driver license and use of a reliable automobile desirable.
- Ability to handle multiple tasks.
- Ability to communicate clearly with others.
- Ability to see and hear in order to interact with others.
- To lift a minimum of 20 lbs.
The target hourly rate for this job is $24.60 - $29.60 with benefits. The specific compensation for this role will be determined based on the education, experience, and skill set of the individual selected for this position.
