DaVita is hiring a Clinical Coordinator to join our outpatient dialysis center. This role plays a crucial part in delivering high‑quality, compassionate care to patients with end‑stage renal disease or chronic kidney disease.

• Coordinate ESRD patient care and services (trend lab values, track metrics on vital signs, weight, outcome management, etc.).
• Monitor patient safety, comfort, and well‑being to ensure the highest level of care.
• Complete training in a facility or training clinic outside of the assigned home clinic.
• Float to various clinics during and after training when needed.
• Maintain a flexible schedule and work mornings, evenings, weekends, and holidays as required.

• Build meaningful, long‑term relationships with patients and their families in an intimate outpatient setting.
• Work within a supportive interdisciplinary team led by RN staff.
• Manage patients with complex comorbidities and utilize broad nursing skills and knowledge.
• Oversee a group of patients with support from Patient Care Technicians (PCTs) for observations, measurements, and machine setup.

• Current Registered Nurse (RN) license in the state of practice.
• Current CPR certification.
• At least 18 months of registered nursing experience.
• Dialysis Charge RN readiness approval by Clinical Nurse Manager or RN‑licensed FA/GFA.
• Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) preferred.
• Experience in ICU, CCU, ER, or Med Surg preferred.
• Associate’s Degree in Nursing (A.D.N) from an accredited school required; B.S.N preferred; diploma from an accredited program accepted.
• Completion of CMS‑required training before operating the water treatment system.
• Supervisory experience preferred; willingness and ability to supervise required.
• Basic computer skills and proficiency in MS Word and Outlook.
