We are currently seeking a compassionate, organized, and team‑oriented individual to join our Home Health team as a Full‑Time or Part‑Time (Per Diem)
Registered Nurse (RN) in Oakland, CA.

We are seeking an experienced, self‑driven Full‑Time or Part‑Time (Per Diem)
Registered Nurse (RN) to provide exceptional care throughout the region, evaluating patients’ health, creating and implementing care plans, and tracking progress to maintain the highest standards of care. You’ll work closely with our Clinical Director and Clinical Staff, providing support both remotely and independently in the field.

• Provide nursing care within scope of practice, following patient’s individualized care plan, in coordination with the interdisciplinary team.
• Administer treatments, monitor patient conditions, and report signs/symptoms indicating a change in condition to Physician, Case Manager, and/or Director of Nursing.
• Maintain accurate clinical records and submit documentation according to company policy using a company‑provided device.
• Supervise Licensed Vocational Nurses (LVNs) when assigned.
• Comply with all company policies and procedures to ensure high standards of care.

• Current Registered Nurse (RN) license to practice in California.
• Minimum 1 year experience in Home Health or Hospice care.
• Current California driver’s license.
• Able to travel within a 15‑25 mile radius on a regular basis.
