Alameda Health System’s Eastmont Wellness Center in Oakland, CA is currently recruiting for a Services As Needed/Per Diem Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our team and work in Adult Medicine.
Eastmont Wellness Center is a large outpatient center that serves a diverse patient population in east Oakland, CA. We provide high quality, accessible care to our community's most vulnerable patients, regardless of their ability to pay for services.
Eastmont offers full scope adult and pediatric primary care and women's health services. We also provide specialty care including orthopedics, ophthalmology, dermatology, rheumatology, podiatry, and more. We have on-site lab, radiology, optometry, dentistry, behavioral health therapy, nutrition services, and case management. Our providers and staff are dedicated to our patients and would love to welcome others who share our mission!
SUMMARYUnder general direction, assesses, evaluates and manages the health care needs of patients. Performs selected diagnostic and therapeutic tasks. Provides health care services and performs clinical procedures that require possession of advanced knowledge and skills and the exercise of a high degree of clinical judgment in the assessment, diagnosis and treatment of patients. Performs related duties as required.
DUTIES & ESSENTIAL JOB FUNCTIONSNOTE:

Any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying.
MINIMUM QUALIFICATIONS Minimum ExperienceDemonstrated experience which would provide the knowledge and skill necessary for successful performance as a Mid-Level Practitioner.
Preferred ExperienceOne year in area of specialty (i.e. Cardiology, Neurology, G.I, Rheumatology, Orthopedics, Hematology, etc.), PG Residency of ED experience or post-graduate training in area of responsibility.
Preferred Licenses/Certification- ACLS, ATLS, SART, dependent on area of specialty.
- BLS - Basic Life Support Certification issued by the American Heart Association.
- PALS - Pediatric Advanced Life Support Certification- AHA American Heart Association (Required for Emergency Department or if overseeing or providing pediatric care on a regular basis).
Successful completion of an approved Nurse Practitioner training program that conforms to the Board of Registered Nursing standards; OR successful completion of a Physician Assistant program of instruction in primary health care approved by the Physician Assisting Examining Committee; OR NCCPA for Physician's Assistant.; graduate of an accredited Nursing program required;
Bachelor’s degree in Nursing preferred.
