Schedule:

Monday - Friday 8am-5pm with every other Friday off

The department is urgently seeking qualified candidates for the Case Manager position. This role supports individuals and families through case management, community engagement, and program coordination to strengthen self‑sufficiency and overall well‑being.

Key Responsibilities
  • Provide case management services in areas such as employment and career development, financial literacy, asset building, leadership development, civic engagement, K–12 and college readiness, health and wellness, youth programs, and senior services.
  • Offer referrals to community service providers and conduct consistent follow‑ups.
  • Manage and coordinate department programs including employment development, financial literacy, leadership initiatives, youth and education programs, and site‑based activities.
  • Lead outreach and recruitment through door‑to‑door engagement, phone calls, and community meetings.
  • Develop marketing, outreach, and educational materials.
  • Record, track, and evaluate program activities and outcomes using case management software and related systems.
  • Collaborate with internal staff on cross‑agency projects.
  • Prepare reports, presentation materials, and program evaluations.
  • Attend and facilitate trainings, workshops, and conferences.
  • Organize departmental, agency‑wide, and community events.
  • Train and supervise staff, volunteers, and interns.
  • Conduct program‑related research and support the drafting of funding proposals.
Qualifications
  • Bachelor’s degree in Social Work, Psychology, Sociology, Human Services
    , Public Administration, or a related field a plus.
  • 2–5 years of experience providing direct services to individuals and families, including case management, community outreach, or program coordination.
  • Experience working with diverse, low‑income, or vulnerable populations in community‑based or social service settings.
