• Schedule: Every other weekend required
• Float: May float to other UCSF facilities (within 75+ mile radius)

• Manage pediatric patients’ care plans from admission through discharge
• Coordinate care with interdisciplinary teams (physicians, therapists, social workers, etc.)
• Perform discharge planning and arrange post‑acute services
• Conduct utilization review and ensure appropriate level of care
• Work with families on education, resources, and transition of care
• Navigate complex cases in NICU/PICU and general pediatrics
• Document all case management activities in the hospital’s charting system
• Ensure safe, timely, and effective patient transitions

• CA RN License (Required)
• AHA BLS (Required)
• COVID‑19 Vaccine + Booster required (Medical/Religious exemptions only)
• Flu Vaccine:

• Experience Requirements: 2 years Case Management experience (Required)
