Overview

The Clinical Social Worker 3 (LCSW) primary responsibilities are psychosocial assessment, crisis intervention, counseling, brief therapeutic interventions, consultation, education, community linkages, supportive services, and to facilitate the plan of care through the health care continuum for patients, families, and staff, with particular focus on high-risk populations. The CSW 3 acts as a consultant to the clinical team, service lines and other departments and participates in program development and quality improvement initiatives.

The CSW 3 will conduct legal and ethical billing for services when applicable and appropriate. The CSW 3 will primarily practice at the Claremont FQHC location (Primary Care/TEEN) but also be available to the School-Based FQHC sites and FQHC Mobile Clinic as needed.

Department Description
At UCSF Benioff Children  Hospital, we understand that hospitalization, illness or injury can be extremely stressful. Clinical Social Workers are available to help patients and their families cope with a broad range of psychological stressors related to illness and being in the medical environment. Our Social Workers are part of the health care team to support patients through the continuum of care.

We strive to reduce health disparities to increase access to care through a trauma informed and anti-racist approach. Our social workers act as patient advocates by serving as counselors, helping to find resources and solving problems. Any concerns related to the psychological, practical and social well-being of each patient or their loved ones may be discussed openly with our social workers.

Responsibilities

The CSW 3 coordinates resources and services in response to a patient’s psychosocial needs while collaborating with care team members to support timely and safe discharges and/or meet the continuum of care needs as best as possible with available resources. The CSW 3 will practice within the scope of UCSF’s mission statement:

Professionalism, Respect, Integrity, Diversity, Excellence. The CSW 3 will participate in department education and practice improvement activities. The CSW 3 has advanced clinical knowledge and skills and is distinguished from the CSW II by leadership activities including participation in advanced program planning, provision of educational programs, clinical supervision, and consultation to the clinical team, other departments, and the community. The CSW 3 will provide social work services to any patient regardless of gender, gender identity, sexual orientation, social, cultural, educational or religious background, language, age, ancestry, citizenship, color, national origin, race, presence of mental or physical disability, lack of source of payment for care, medical condition, marital status or status as a veteran.

REQUIRED FOR CLINICAL SOCIAL WORKER II

  • Masters degree from a CSWE accredited school of Social Work.
  • Experience (internship/career) in a healthcare and/or hospital setting preferred.
  • Knowledge and abilities essential to the successful performance of the duties assigned to the position, including knowledge of relevant community resources commensurate with illness, symptoms, mental status, and impairments in functioning and knowledge of the medical and psychological sequelae of course of illness and impact on social functioning (may be specific to patient population).
  • Experience in crisis de-escalation and trauma informed care.
  • Demonstrates resourcefulness, effective written and oral communication, diplomacy, and organizational and analytic skills.
  • Self-directed, assertive and creative in problem solving, systems planning and patient care management in a high-volume work environment.
  • Ability to work effectively and collaboratively with interdisciplinary teams.
  • Basic pager, telephone, and computer skills including the use of the internet.
  • This position requires flexibility to orient and work at all UCSF Medical Center locations.
