Job Summary

The Health Coach is a Medical Assistant who, under supervision from the Clinic Site RN Manager or Clinic Units Manager, also serves as a member of the chronic care management team. They collaborate with patients and their providers to optimize the delivery of health care services, provide medical assistance and interpretation, coordinate patient care with providers, and perform clerical tasks.

Essential Job Functions
  • Provide the highest quality Medical Assistant care according to clinic policies and culturally competent care aligned with the agency’s Guide to Service Excellence and Corporate Compliance Plan.
  • Safeguard patients’ privacy rights and cooperate in the resolution of patient complaints.
  • Participate in a year-long intensive training program to learn the core competencies required of a Health Coach II; during training, perform all Health Coach II duties under close supervision with the goal of independent work by the end of the period.
  • Take and record vital signs, recognize abnormalities, and report them to nurses and providers.
  • Interview patients to obtain and record medical history using SOAP format in preparation for medical visits.
  • Obtain basic health information and relevant documents and records.
  • Participate in team huddles with providers before the start of the clinic day to prepare for patient visits and manage standing orders.
  • Perform basic in-clinic lab tests and obtain/process specimens for lab send-out; conduct tests such as electrocardiogram, spirometry, audiometry, and Snellen chart.
  • Perform basic nursing procedures such as injections (immunizations and tuberculin skin tests only), phlebotomy, and dressing changes.
  • Fill out forms for ancillary services or referrals as directed by a physician.
  • Assist in optimizing patient flow.
  • Perform sterilization, infection control, and cleaning of clinic areas and medical instruments/equipment.
  • Maintain and order clinic supplies, keeping examination rooms and nursing areas well-stocked.
  • Perform interpretation duties and make simple referrals as required or assigned.
  • Enter data and retrieve reports from the CDMS system as needed.
  • Meet with provider staff to discuss elements of patient care and carry out orders as directed.
  • Enter health information into electronic health records (EHR) and utilize EHR for documentation, records retrieval, staff communications, and data reporting.
Technical/Ethical
  • Adhere to policy and procedures within the unit.
  • Maintain patient rights to privacy, safety, and confidentiality.
  • Attend appropriate in-service education courses for technical growth.
Leadership
  • Attend all staff and component meetings, providing input as necessary; may participate in AHS subcommittees.
  • Identify problems in the work area and collaborate to seek solutions.
  • Demonstrate good communication skills with patients.
  • Support AHS organizational goals and participate in orientation and training for volunteers and new staff.
Minimum Qualifications
  • Experience as a clinical Medical Assistant with education and training in the Medical Assistant field, delivering proficient and culturally compassionate care.
  • Minimum one year of clinical experience in a healthcare setting.
  • Bilingual/bicultural in an Asian language required — Cantonese, Mandarin, Korean, Khmer, Burmese, or Vietnamese.
Certifications, Licenses, Registrations
  • Will be required to take the LCAP Medical Interpretation course and exam within 12 months of employment.
  • CPR certification required.
  • Medical Assistant certification must be obtained within one year of hire as a Health Coach I.
