La Clínica de La Raza is a community-based health center committed to providing culturally appropriate, high-quality, and accessible health care to the diverse communities of the San Francisco Bay Area. We have spent over 40 years advocating for and creating a health home for the many that have been denied access to care. As health care activists, we are dedicated to making sure individuals who do not have health coverage get the same level of quality healthcare as those who have it.
From our genesis as a community health center in 1971, we understood that we cannot heal our communities without also addressing the economic and social factors that affect health. We coordinate and connect patients to a broad network of services to improve and maintain their health and well-being across 35 sites in Alameda, Contra Costa and Solano counties.

Major Areas of Responsibility
Healthcare Delivery
- Provide all aspects of outpatient pediatric care to children from birth through adolescence
- Provide routine, acute, urgent, preventive, curative, and rehabilitative medical care to youth, including initiating referrals to medical specialists as required
- Screen and monitor the management of stabilized chronic conditions in asymptomatic and well patients
- Attend to and/or admit patients to appropriate hospitals as needed while ensuring 24-hour availability of care by sharing night and weekend call with other physicians
- Other duties as assigned by supervisor
Patient Experience
- Deliver culturally sensitive well and chronic disease care to a diverse patient population of all ages
- Demonstrate flexibility in meeting patients’ needs and participate in after-hours call pool and Saturday clinics rotation
- Develop a positive rapport with patients and families to foster the physician/patient relationship
Collaboration & Mentorship
- Serve as a member of the cross-functional team of providers, consulting with Nurse Practitioners and Physician Assistants on assessment, management and treatment of complicated cases, chart review, co-signing, and case conferences
- Suggest improvements for clinic flow, patient scheduling and service delivery; collaborate with interdisciplinary team members including providers, health educators, medical assistants, registered nurses, and case managers
- Provide direction to medical assistants regarding patient care as needed
Minimum Job Requirements
Knowledge
- Knowledge and experience with Electronic Health Record programs, such as Next Gen or Epic
- Knowledge of the information and techniques needed to diagnose and treat human injuries, diseases, and deformities, including symptoms, treatment alternatives, drug properties and interactions, and preventive health-care measures
- High degree of interpersonal competence with both staff and patients, and thorough knowledge of evidence-based care
- Cultural competence working in diverse/low income communities
