RN Case Manager – Home Health Care

We are seeking a compassionate and dedicated Registered Nurse to provide high-quality care to patients in the comfort of their own homes. The ideal candidate will be responsible for managing a range of nursing duties, ensuring the delivery of individualized care plans, and fostering positive relationships with patients and their families.

• Conduct comprehensive intake health assessments and supervisory visits to assess patient needs and develop personalized care plans.
• Administer medications, set up medication regimens, provide treatments, and monitor patient progress, adjusting care plans as necessary.
• Educate patients and their families on effective healthcare techniques, disease management, and self-care practices to promote optimal health outcomes.
• Collaborate with healthcare professionals to ensure coordinated, holistic patient care and seamless communication.
• Maintain accurate and up-to-date patient medical records, documenting interactions, interventions, and patient progress in a timely manner.
• Monitor and report any changes in patient conditions, symptoms, or progress to the care team and make appropriate recommendations.
• Create a safe and comfortable home environment for patients, providing emotional support to both patients and their families.
• Utilize eRSP software to schedule and track patient visits, ensuring timely and efficient service delivery.

• Graduate of an accredited school of nursing.
• Valid unencumbered RN license in the state of Minnesota.
• Minimum of 1 year of experience in home healthcare, hospital, or similar clinical settings.
• Ability to work independently, manage time effectively, and prioritize patient care needs.
• Proficiency in using electronic medical record (EMR) systems.

  : $62.00 per visit

• Earned Safe and Sick Time (ESST)
• This is a per diem position and is not eligible for benefits

• Visits generally are scheduled between 8am - 5pm; times are coordinated between RN and the client.
• Flexible schedule with no weekend or evening shift requirements.

• Must be a licensed driver with a reliable automobile that is insured in accordance with state and/or organization requirements.
• Position located in the Minneapolis area.
