- Provide total nursing care to residents on the assigned unit during the shift as a Licensed Practical Nurse (LPN)
- Participate in developing and implementing individualized care plans alongside the healthcare team
- Administer medications and treatments as prescribed and evaluate effectiveness regularly
- Maintain an environment that promotes residents’ physical, social, and psychological well-being
- Educate residents and families on care plans and medical procedures
- Monitor team member performance and assist with development, evaluation, and corrective action if needed
- Delegate and ensure completion of assigned nursing tasks to team members
- Stay current with facility policies, procedures, and ongoing nursing education
- Recognize and act on significant changes in resident conditions
- Document care, responses, and relevant information per facility guidelines
- Act as a charge nurse: answer questions, resolve issues, and support resident care decisions
- Follow all facility safety policies and procedures
- Graduation from an accredited nursing education program
- Active Licensed Practical Nurse (LPN) license in the state of Illinois
- No prior abuse convictions
- Strong communication and teamwork skills
- Ability to follow policies, maintain safety, and deliver empathetic care
- Must be able to work with diverse resident populations, including elderly and mentally ill individuals
- Must be physically able to perform nursing duties with or without accommodations
