Base pay range: $21.00/hr - $24.00/hr

• Provide social services to residents, including emotional support, counseling, and advocacy
• Assist in assessing and identifying the social and emotional needs of residents
• Develop and implement individualized care plans to meet the unique needs of each resident
• Collaborate with other healthcare professionals to ensure comprehensive care for residents
• Maintain accurate and timely records of resident interactions and progress
• Participate in community outreach and education initiatives to promote the values and mission of ALIYA
