REGISTERED NURSE – HOME HEALTH
OverviewLocation: Southeast City, 60620 Auburn, Gresham, Brainerd, Princeton Park, Beverly View; 60628 Washington Heights, Roseland, Cottage Grove Heights, Pullman; 60643 East Beverly, Beverly, Morgan Park
Schedule: Monday–Friday, 8:00 a.m.–5:00 p.m., with every 4th weekend rotation, shared after-hours call, and two holidays per year.
Sign-On Bonus: Up to $10,000. This position is eligible for up to a $10,000 sign-on bonus. You must be actively working in this same position at the time of payout. Current internal team members are not eligible at this time.
This position is with Advocate Home Health Services, part of Advocate Health Care. The RN will provide home care services to the homebound population and establish strong relationships with patients and caregivers to assist the patient in reaching their maximum potential.
Our team helps patients and caregivers become independent in managing conditions with high-quality nursing care to support home-based treatment and prevent rehospitalization. All medical needs will be coordinated by the RN who acts as the case manager and utilizes the resources of the team (e.g., social worker, CNA, MD).
Key Responsibilities- Assumes primary responsibility for the assessment, planning, implementation, and evaluation of nursing care for assigned patients in their homes
- Establishes patient-centered goals using a holistic approach that supports patients and families
- Provides leadership at the unit level and serves as a clinical resource
- Demonstrates strong clinical judgment and mastery of nursing skills
- Nurse/Patient Relationship: Develops and maintains therapeutic relationships with patients and caregivers throughout the healthcare continuum.
- Assuring/Improving Quality Care: Promotes the delivery of high-quality, cost-effective care while advocating for patient and family rights.
- Patient and Staff Education: Demonstrates a commitment to meeting the educational needs of patients, families, and colleagues. Uses resources to support learning and positive patient outcomes. Fosters professional growth.
- Nursing Process: Applies the nursing process as a systematic, problem-solving approach to patient care.
- Leadership, Collaboration, and Professional Development: Works collaboratively with interdisciplinary team members to provide integrated care. Demonstrates leadership behaviors and commitment to professional development.
- Graduate from an accredited school of nursing
- One year of medical-surgical experience in an acute or subacute care facility
- Current Illinois RN license
- Current CPR certification
- Earn 10.0 continuing education credits annually
- Ability to lift up to 35 pounds without assistance; for lifts over 35 pounds or when the patient cannot assist, use patient handling equipment with at least one other associate when available; assess unique lifting situations case-by-case. Ability to assist with ambulation including turning and transferring patients up to 200 pounds
$74,672.00 – $ annual base salary
