Position Purpose
The Home Health WOCN RN provides expert nursing care in wound, ostomy, and continence management to home care patients. They collaborate with healthcare professionals and patients/families to ensure optimal outcomes.
- Graduate of an accredited nursing program (BSN preferred)
- Current RN license in the state of practice
- Certified Wound Ostomy Continence Nurse (CWOCN) certification required
- Minimum 2 years of RN experience in a clinical setting
- Proficiency in electronic medical records systems
- Strong assessment, critical thinking, and communication skills
- Ability to work independently and as part of a team
- Valid driver's license and reliable transportation
- Current CPR/BLS certification
- Conduct comprehensive assessments of wounds, ostomies, and continence issues
- Develop individualized care plans with the interdisciplinary team
- Educate patients and families on wound care, ostomy care, and continence management
- Provide direct patient care and monitor progress
- Participate in quality improvement activities and maintain documentation
- Provide in-service education and participate in continuing education
