Position Purpose
The Home Health WOCN RN provides expert nursing care in wound, ostomy, and continence management to home care patients. They collaborate with healthcare professionals and patients/families to ensure optimal outcomes.

• Graduate of an accredited nursing program (BSN preferred)
• Current RN license in the state of practice
• Certified Wound Ostomy Continence Nurse (CWOCN) certification required
• Minimum 2 years of RN experience in a clinical setting
• Proficiency in electronic medical records systems
• Strong assessment, critical thinking, and communication skills
• Ability to work independently and as part of a team
• Valid driver's license and reliable transportation
• Current CPR/BLS certification

• Conduct comprehensive assessments of wounds, ostomies, and continence issues
• Develop individualized care plans with the interdisciplinary team
• Educate patients and families on wound care, ostomy care, and continence management
• Provide direct patient care and monitor progress
• Participate in quality improvement activities and maintain documentation
• Provide in-service education and participate in continuing education
