Ready to join? We are looking for a full-time Outpatient Physical Therapist to join our rehabilitation team. Our PTs plan, implement and evaluate patient care programs designed to increase, maintain or restore function, minimize or prevent disability and assist patients in reaching their maximum performance levels.
Base pay range$80,000.00/yr - $/yr
Essential Duties and Responsibilities- Evaluates patient and gathers appropriate information to determine accurate clinical diagnoses and treatment plan.
- Establishes appropriate, functional and objective goals based upon clinical examination.
- Devises detailed treatment and care plans for patients.
- Provides exceptional patient care and service.
- Maintain friendly, cordial relations with all clients and employees; maintain a positive work atmosphere by acting and communicating in a manner that results in positive and respectful working relationships.
- Cultivate relationships with patients and the community.
- Provide continuous communication with referring physicians and other referral sources to ensure a consistent level of care.
- Maintain patient charts, clinical logs and checklists necessary for compliance.
- Maintain patient confidentiality regarding medical records per company policies and procedures and ensure compliance with HIPAA guidelines.
- Communicates well in a professional setting.
- Perform additional duties as required.
- Bachelor’s, Masters or Doctorate Degree from an accredited Physical Therapy Program
- Current State of Illinois Physical Therapy license (or willingness to apply)
- CPR Certified
