We have an exciting opportunity for three Adult Social Workers to join the innovative Transformation Team in Nottingham

This position is ideal for experienced practitioners with strong frontline experience and Care Act assessment skills who are ready to support service improvement and transformation.

What You Will Do:

Work within a proactive Transformation Team focused on enhancing adult social care services.

Complete Care Act assessments and ensure timely, high-quality outcomes.

Manage and review care packages across Learning Disabilities and Mental Health services.

Use Liquid Logic to maintain accurate, up-to-date case records and documentation.

Attend team meetings and contribute to service improvement strategies.

Collaborate with multi-disciplinary professionals to deliver integrated, person-centred support
.
