Job Summary
We have an opportunity for a Band 5 Community Nurse to join Nottingham City Care working full time, Fixed Term until September 2026.
The Band 5 Community Nurse role at City Care offers an excellent opportunity to develop clinical skills across a wide range of community settings. Community Nurses play a vital role in delivering high quality, person‑centred care to patients in their own homes and community settings, focusing on supporting patients to maintain independence, manage long‑term conditions and avoid unnecessary hospital admissions through pro‑active, holistic nursing care.

Key Responsibilities- Provide skilled nursing care including wound management, medication administration, palliative and end‑of‑life care, catheter care and chronic disease management.
- Undertake comprehensive assessments and develop evidence‑based care plans tailored to individual patient needs.
- Work collaboratively with multidisciplinary teams including GPs, therapists and social care professionals to ensure continuity and coordination of care.
- Promote health education, self‑management and independence for patients and carers.
- Maintain accurate, timely documentation in line with NMC and organisational standards.
- Participate in clinical supervision, reflective practice and ongoing professional development.
- Contribute to service improvement initiatives and uphold compassionate, safe and effective care within the community.
- Contribute to public health agenda by raising awareness of health and well‑being needs of individuals, promoting prevention and early intervention.
- Plan, deliver and evaluate programmes of care to address peoples health and well‑being needs.
- Deliver evidence‑based care.
- Seek patient and carer feedback about the service.
- Maintain professional standards as outlined in NMC code.
- Apply clinical governance agenda and risk management systems.
- Participate in research and audit activity.
- Take responsibility for appropriate delegation of nursing care to team members.
- Maintain record‑taking practice in line with City Care, NMC and professional standards.
- Communicate and liaise with individuals and groups across a wide variety of settings.
- Act as a mentor to nursing students and support the development of other team members.
- Maintain portfolio to demonstrate continuing professional development.
- Promote best practice ensuring health, safety and security of self, patients and colleagues in accordance with NCCP policies and guidelines.
- Practice in accordance with safeguarding legislation and guidelines.
- In the absence of District Nurse Team Leader or deputy, take responsibility for the needs of patients and carers on the community nursing caseload.
- Essential:
Active nursing qualification with the NMC. - Desirable:
Post‑registration qualifications relevant to community nursing.
- Essential:
Up‑to‑date clinical knowledge including wound care, continence management, palliative care, equality and diversity, effective communication skills, basic IT skills, time management and organisational skills, teamwork skills, customer service skills, positive approach to change. - Desirable:
Knowledge of community wound care, continence and palliative care; ability to work unsupervised; ability to motivate others.
- Essential:
Up‑to‑date clinical knowledge (wound care, continence, palliative care). - Desirable:
Postgraduate experience in a primary care setting; post‑registration experience or completion of a community management placement; evidence of working in a team.
- Ability to be flexible over hours worked within contracted hours.
