The Linden Medical Group is seeking a motivated and organised QOF & Practice Administrator to join our friendly and supportive primary care team.
This role combines Quality and Outcomes Framework (QOF) administration with general practice administrative duties. The successful candidate will play a key role in supporting clinical teams, maintaining accurate patient records, managing recall systems, and ensuring the practice meets contractual and governance requirements.
Main duties of the job- Manage and maintain QOF and clinical data using the Systm One clinical system.
- Monitor QOF indicators, generate reports, and support the practice in achieving QOF targets.
- Ensure all eligible patients are recalled for annual QOF and chronic disease reviews.
- Manage non-QOF recalls, including DOAC monitoring and secondary care initiated recalls.
- Process clinical correspondence and pathology results in line with practice procedures.
- Download, attach, code, and forward clinical letters to the appropriate clinicians.
- Use MJOG SMS and other systems to invite patients for reviews and appointments.
- Support audits, inspections, and reporting requirements.
- Work closely with the Practice Manager and clinical team to improve performance and patient outcomes.
- Ensure administrative SOPs are followed and kept up to date.

Key Responsibilities
QOF and Data Management
- Maintain accurate, complete, and up‑to‑date patient records using the Systm One clinical system.
- Input, review, validate, and code QOF‑related clinical data in line with national and contractual requirements.
- Generate, analyse, and interpret QOF and performance reports to identify gaps and areas for improvement.
- Monitor QOF indicators proactively and take appropriate action to support achievement of targets.
- Ensure all patients eligible for QOF reviews are identified, recalled, and followed up annually.
- Manage and oversee patient recall systems for chronic disease management, QOF, and non‑QOF recalls (e.g. DOAC monitoring).
- Add and maintain recalls triggered by secondary care instructions.
- Use MJOG SMS and other communication methods to invite patients for reviews and track responses.
- Monitor uptake and attendance, escalating concerns where required.
- Download and process clinical correspondence via GP Access and practice email inboxes.
- Attach letters to patient records, apply appropriate clinical coding, and forward to the relevant clinician.
- Distribute incoming pathology results in accordance with practice protocols.
- Prepare and submit QOF progress and performance reports for internal review and external audit purposes.
- Support clinical audits, inspections, and contractual reviews as required.
- Keep administrative Standard Operating Procedures (SOPs) accurate and up to date.
- Remain up to date with changes to QOF, contractual requirements, and NHS guidance, ensuring the practice remains compliant.
- Work closely with the Practice Manager to develop and implement strategies to maximise QOF achievement.
