Job OverviewAn exciting opportunity for experienced Mental Health Nurses to work within a community setting. The successful candidate will develop skills to support adults across the area, including secondary care services within SMI pathways (mood affective, complex psychosis, community rehabilitation and non‑psychosis). You will receive comprehensive training, regular supervision and support within a very supportive team environment.
Responsibilities- Hold a caseload and work with patients and their families to support personalised care plans and provide specialist interventions.
- Promote recovery and collaborate with partner agencies and primary care to ensure timely and safe discharge from secondary care.
- Complete both triage assessments and detailed initial assessments of new referrals.
- Provide support via the team’s duty system, either by telephone or face‑to‑face.
- Act as a leadership role, developing specialist skills within at least one pathway and supporting students and junior staff through supervision and mentorship.
- May be asked to line‑manage some staff.
