Job Summary

Join Our Friendly Team at Jubilee Park Medical Partnership is a welcoming GP practice serving 12,300 patients across two sites in Carlton and Lowdham. We’re looking for three enthusiastic, flexible, and friendly team members to join our Care Coordination Team.

Join Our Front Desk Team as a Care Coordinator

As the first point of contact for our patients, you’ll be the friendly face and reassuring voice they rely on. You’ll be at the heart of our practice, ensuring everything runs smoothly by:

  • Welcoming patients with warmth and assisting them in person or over the phone
  • Handling reception and administrative duties with ease and professionalism
  • Collaborating with healthcare professionals to provide seamless care
  • Managing patient appointments and directing them to the appropriate services
  • Offering exceptional customer service with a positive, problem‑solving mindset

Job Description

To be responsible for undertaking a wide range of reception duties and the provision of general support to the multidisciplinary team. Duties can include but are not limited to greeting and directing patients, effective use of the appointment system, booking appointments, processing of information and assisting patients as required. To act as the central point of contact for patients, the distribution of information, messages and enquiries for the clinical team, liaising with multidisciplinary team members and external agencies such as secondary care and community service providers.

Flexibility to work across sites is essential.

Person Specification Qualifications

Essential

  • Educated to GCSE level or equivalent
  • GCSE Mathematics & English (C or above)

Desirable

  • Customer service qualification (NVQ) or equivalent
  • NVQ Level 2 in Health and Social Care
Skills

Essential

  • Excellent communication skills (written and oral)
  • Strong IT skills
  • Clear, polite telephone manner
  • Competent in the use of Office and Outlook
