• Tri Health Primary Care – at 1775 W Lexington Ave

• Full‑Time (80 hours bi‑weekly)
• Shift varies – 8:00 AM – 4:30 PM
• Evening and weekend rotation

• Associate's Degree or Diploma in Nursing
• For Breast Center positions, Certified Breast Care Nurse (ONCC) is required within one year of hire
• Basic Life Support for Healthcare Providers (BLS) Registered Nurse
• Current Ohio licensure
• Knowledge of the specialty or population served
• Ability to work effectively on a multi‑disciplinary team
• 1–2 years experience in Nursing

This position provides nursing services, either directly or indirectly, to participants in a specific program, specialty department, or practice. It includes initial and regular re‑assessments, ongoing nursing services, and interdisciplinary team collaboration. In inpatient settings, an RN may write verbal orders, enter orders, pend access notes, and attend physician THC training.

• Plan care for participants and families based on assessment, standards of care, and optimal outcomes. Initiate and individualize patient care guidelines, POC, or clinical pathways. Maintain current plan of care by updating as needed. Develop short and long‑term goals. Participate as a member of an interdisciplinary team, identifying caregiver and participant needs, recommending interventions, and collaborating with other health‑care team members for mutually determined solutions.
• Perform technical skills per hospital and department policies, procedures, appropriate competencies, and accepted standards of practice. Implement POC. Prioritize workload according to participant needs and requests. Base interventions on clinical data, desired outcomes, the nursing process, and standards of care. Document interventions and outcomes. Utilize critical thinking skills in daily care, including accurately receiving, interpreting, and executing physician orders.
• Evaluate the plan of care based on optimal patient outcomes. Document participant/family response to care, including teaching. Collaborate with other members of the health‑care team to revise the plan of care as needed. Supervise delegated care to other health‑care team members. Demonstrate organization responsibilities, identify areas for self‑improvement, function in relief charge/resource roles as requested, complete assignments within scheduled time frames, cooperate with instructors to facilitate effective learning experiences for students, maintain current knowledge, and demonstrate knowledge of organizational and unit changes.
• Conduct and document comprehensive patient assessments with emphasis on promotion of health, prevention, and early detection to plan and implement care directed toward optimal specific outcomes. Include health counseling and teaching needs in assessments. Communicate assessment findings to other health‑care providers as appropriate.
