Make a Real Impact as a Home Health Regional Floater RN

Location: Norwood, MA
Compensation: $90,000 – $110,000 annually

Are you an experienced and compassionate Registered Nurse looking for a leadership opportunity with flexibility and purpose? We are seeking a skilled RN Patient Care Manager (PCM) to serve as a Regional Floater within our Home Health division. This role is ideal for a dynamic clinician who thrives in a fast‑paced environment and is committed to delivering exceptional patient care across multiple locations in the Norwood, MA region.

As a key member of our interdisciplinary team, you’ll play a vital role in ensuring clinical excellence, supporting field staff, and guiding patients and families through the home health journey.

Key Responsibilities
  • Oversee and coordinate patient care services across various home health sites within the region

  • Conduct comprehensive assessments and develop individualized care plans

  • Ensure clinical compliance with state, federal, and Medicare regulations

  • Provide mentorship and leadership to nursing and field staff

  • Collaborate with physicians, caregivers, and interdisciplinary team members to optimize patient outcomes

  • Monitor patient progress and adjust care plans as needed

  • Support quality assurance initiatives and participate in case conferences

Qualifications
  • Active Registered Nurse (RN) license in the state of Massachusetts

  • Minimum 2 years of supervisory experience in home health or a related healthcare setting preferred

  • Strong working knowledge of Medicare guidelines

  • Exceptional communication, organizational, and problem‑solving skills

  • Ability to travel regionally between patient locations and branch offices
