Compensation: $90,000 – $110,000 annually
Are you an experienced and compassionate Registered Nurse looking for a leadership opportunity with flexibility and purpose? We are seeking a skilled RN Patient Care Manager (PCM) to serve as a Regional Floater within our Home Health division. This role is ideal for a dynamic clinician who thrives in a fast‑paced environment and is committed to delivering exceptional patient care across multiple locations in the Norwood, MA region.
As a key member of our interdisciplinary team, you’ll play a vital role in ensuring clinical excellence, supporting field staff, and guiding patients and families through the home health journey.
Key ResponsibilitiesOversee and coordinate patient care services across various home health sites within the region
Conduct comprehensive assessments and develop individualized care plans
Ensure clinical compliance with state, federal, and Medicare regulations
Provide mentorship and leadership to nursing and field staff
Collaborate with physicians, caregivers, and interdisciplinary team members to optimize patient outcomes
Monitor patient progress and adjust care plans as needed
Support quality assurance initiatives and participate in case conferences
Active Registered Nurse (RN) license in the state of Massachusetts
Minimum 2 years of supervisory experience in home health or a related healthcare setting preferred
Strong working knowledge of Medicare guidelines

Ability to travel regionally between patient locations and branch offices
