Overview

The MDS Coordinator plays a vital role in ensuring the accurate and timely completion of the Minimum Data Set (MDS) assessments for patients within our healthcare facility. This position is essential for maintaining compliance with federal and state regulations while promoting high-quality patient care. The ideal candidate will possess a strong background in medical documentation, outpatient services, and emergency medicine, along with proficiency in electronic health record systems;
Point Click Care experience is a plus.

Duties
  • Conduct comprehensive assessments of patients to complete MDS documentation accurately.
  • Collaborate with healthcare professionals to gather necessary medical records and patient information.
  • Ensure compliance with all regulatory requirements related to MDS submissions.
  • Maintain up-to-date knowledge of changes in regulations affecting MDS processes.
  • Facilitate interdisciplinary team meetings to discuss patient care plans and outcomes.
  • Monitor and analyze data related to patient care quality metrics.
  • Provide training and support to staff on MDS-related processes and documentation standards.
  • Utilize electronic health record systems effectively to manage patient information.
Qualifications
  • Proven experience in a medical office or healthcare setting;
    Skilled Nursing Facility experience is a must.
  • RN or LPN license active in the state of Massachusetts.
  • Strong knowledge of medical documentation practices and patient care protocols.
  • Familiarity with electronic health record systems such as Point Click Care is highly desirable.
  • Excellent organizational skills and attention to detail for accurate data entry and record keeping.
  • Ability to work collaboratively in a team-oriented environment while managing multiple priorities.
  • Strong communication skills, both verbal and written, for effective interaction with patients and staff.
  • Background in emergency medicine is a plus, enhancing the understanding of urgent patient needs.
