Responsibilities- Assist with all hands‑on direct care (showering, toileting, shaving, brushing teeth, meal‑time assistance)
- Ensure health and safety of the individuals, including personal care
- Provide education, training, counseling, and physical assistance to achieve ISP goals in functional academics, domestic skills, health and personal hygiene, social and leisure skills, communication skills, community mobility, self‑esteem, and money management
- Communicate with family, friends, guardians, advocates, volunteers, and employers to facilitate consistency of programming and support
- Develop, write, and present annual ISPs and semiannual agreements in consultation with residential staff, the program manager, and the director
- Perform case‑management duties
- Provide transportation to individuals as necessary
- Schedule and supervise staff
- Experience working with individuals with /DD
- Bachelor’s degree or associate degree in a related field with two years of experience, or a high‑school diploma and five years of related experience in human services
- Must be able to pass fingerprint background check, CORI, DPPC, and driving background checks
- Valid driver’s license required
- Valid MAP certification preferred
- Excellent verbal and written communication skills
