Role Overview
Our client isseeking adedicated Care Coordinator to join their team, focusing on office-based coordination and support for care staff. This role is ideal for someone highly organised and passionate about ensuring smooth care delivery.
Key Responsibilities
- Manage and coordinate care rotas to ensure efficient service delivery
- Provide ongoing support to care staff and assist with scheduling
- Onboard new clients, ensuring a full understanding of individual needs before handover
- Drive care staff to live-in placements and clients to appointments when necessary
- Support waking nights when required
- Provide compassionate support for ‘end of life’ patients
Requirements
- Strong organisational and communication skills
- Ability to work flexibly and adapt to changing priorities
- Full UK driving licence
