Position Summary: The Nurse Practitioner/Physician Assistant provides comprehensive outpatient care to individuals diagnosed with cancer, from diagnosis through treatment during the acute phase of survivorship. The NP/PA is an essential member of the multidisciplinary clinic team, including the survivorship dietitian and exercise specialist. This position is responsible for coordinating care and ensuring appropriate referrals to ancillary and supportive resources to address physical, psychosocial, and other identified needs.

The Advanced Practice Provider primarily provides outpatient clinic care at our new Ambulatory Cancer Care Building in Central West End, St. Louis, with potential to float to county outreach facilities including Siteman South County, Siteman Barnes St Peter's, Siteman North County, Siteman – Shiloh, IL, and Siteman West County. This position offers a hybrid workflow, with the possibility of work-from-home days for administrative responsibilities on non-clinic days.

Duties & Responsibilities
  • Evaluates survivors’ needs (physical, psychological, and social) using assessment tools such as distress screening to connect patients to referrals and support services (e.g., dietitians, social work, physical therapy).
  • Provides psychosocial support and facilitates referrals to psychology services as needed.
  • Adheres to NCCN survivorship clinical guidelines and other evidence-based practices.
  • Collaborates with the Survivorship Program Team to curate and develop educational materials for survivors.
  • Provides direct patient care, including obtaining patient history, performing assessments, ordering, performing and interpreting diagnostic studies, and providing preventive health assessments, screenings, immunizations, and care; develops and implements a patient-centered plan of care in collaboration with physicians, patients, consultants, nurses, and other providers as appropriate.
  • May bill for services covered by collaborative practice agreement.
  • Provides counseling and education to patients and families/caregivers concerning disease management, complication prevention, disease prevention, quality of life improvement, and outcome management activities.
  • Monitors accuracy of patient medical records; maintains required reports, records, statistics, etc.
  • Maintains a safe, comfortable and therapeutic environment for patients/families in accordance with clinic/hospital standards.
  • Manages health conditions via prescriptive authority or referrals to other providers or community resources; may perform diagnostic or therapeutic interventions consistent with certification and state regulations.
  • Champions the safety and quality of care for patients using ITB therapy and serves as an expert in ITB pumps; helps develop educational programs and lead quality/safety initiatives for the program.
  • Serves as clinical expert regarding rehabilitation nursing with autonomy and expertise in complex clinical situations.
  • Provides management services for clinical research study projects; monitors participating subjects including medication administration and education.
  • Works in accordance with departmental policies, quality assurance, safety, environmental and infection control standards.
  • Engages in continuous quality improvement and process improvement activities to provide exceptional, evidence-based patient care.
  • Supports professional growth through educational programs, literature, in-service meetings and workshops.
  • Contributes to advancement of the rehabilitation nursing profession through research, publications, and presentations.
  • Performs other duties as assigned.
