We are Brighton/Suncrest Hospice, a CHAP certified hospice in Chicago, IL. Our goal is to provide exceptional care and service to patients by improving staff‑to‑patient ratios and clinical visits while lowering clinician caseloads.

• Provide care and services to a group of patients, visiting patients in private homes, Skilled Nursing Facilities, and assisted living facilities.
• Collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Work independently to manage visits, documentation, meetings, and communication with multiple healthcare professionals.
• Manage time effectively to schedule visits, attend required meetings, and prioritize urgent needs.
• Learn and use an electronic medical records system.
• Drive with reliable transportation, valid driver’s license, and auto insurance to travel across the service area.

• Current registered nurse (RN) licensure in good standing in the state of practice.
• Strong teamwork skills to collaborate daily with interdisciplinary team members.
• Ability to work independently and manage day-to-day activities including phone, email, written communication, and meetings with various healthcare professionals.
• Time management skills to prioritize and triage multiple concerns.
• Willingness to drive with reliable transportation, valid driver’s license, and auto insurance.
