Excelcare Home Health is seeking an experienced Home Healthcare Consultant to assist with rapid census development and referral source rebuilding for a licensed home health agency in Northbrook, IL.
Scope of Work
- Rebuild and activate referral sources (physicians, hospitals, discharge planners, ALFs, SNFs)
- Develop and execute census growth strategy
- Assess current marketing/referral gaps and provide actionable recommendations
- Support intake and admissions workflow as needed
- Leverage existing referral relationships (strongly preferred)
Requirements
- Strong knowledge of Illinois home health market
- Ability to start immediately
- Results-driven and self-directed
- Short-term or project-based (with potential extension)
- Competitive compensation (hourly or performance-based)
