RN Field Case Manager –
Lightways Hospice and Serious Illness Care

Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness is an independent, non‑profit healthcare provider licensed in 11 counties in northwest Illinois. We have a state‑of‑the‑art facility and the first free‑standing in‑patient Hospice Home in Illinois. We have a 35‑year history of providing compassionate professional care to terminally ill patients and their families. We are state licensed and Medicare/Medicaid certified.

We are seeking a full‑time RN for Lake and Cook counties to provide care for patients residing in their private home, nursing home or long‑term care facility. The RN Case Manager will manage a caseload of patients, visit them according to their plan of care, and provide loving care for activities of daily living. The position works Monday‑Friday and requires a minimum of 20 hours of on‑call per month.

• Provide direct nursing care and support for patients in their home or long‑term care settings.
• Manage patient caseloads, coordinating care plans and daily activities.
• Collaborate with interdisciplinary teams to ensure continuity of care.
• Document care activities and communicate progress to patients, families, and providers.

• Illinois Registered Nurse license.
• Minimum of one year of experience in home health or nursing home/long‑term care.
• Previous hospice experience is helpful.
