Responsibilities

The post-holder will be informed of the various aetiologies underlying heart failure and the evidence-based management strategies employed to improve an individual's quality of life. The post-holder is supported in reviewing patients referred to the Heart Failure team within both the acute setting, post‑discharge and chronic disease management setting. In the acute setting a virtual ward will be offered, allowing suitable patients to be managed at home with a combination of remote monitoring or home visits as required.

The post-holder must have a valid driver’s licence and access to a vehicle for work. In the chronic setting the role includes patient reviews within a clinic setting, delivering individualised care, facilitating lifestyle changes through education and support to patients, carers and other healthcare professionals. The post-holder will work towards non‑medical prescribing competency with a view to initiation and up‑titration of heart‑failure guideline‑directed medical therapies.

The post-holder also provides a direct assessment telephone service, prioritising and appropriately directing individualised care. The post-holder will work in collaboration with other members of the heart‑failure service, being supported by the Matron for Cardiology, Heart Failure Consultant Lead, Heart Failure Consultant Cardiologists and the HFSN Team. Depending on service needs, the post-holder may work across Kettering and Northampton hospital sites.

Additional Information

Applicants who require a Disclosure Barring Service (DBS) check, as a requirement of the role, will be required to repay the cost of obtaining a DBS check (£49.50) and this amount will be reclaimed from your first salary. From 1 February 2019 all new starters are required to join the DBS update service as per the Trust DBS Re‑checking Policy (annual cost £16).

New employees who do not join the update service will need to pay £49.50 for a new DBS check in three years time. Please note that new starters with KGH are subject to a six‑month probationary period. Please ensure that the information you provide on your application form is correct, accurate and that nothing has been omitted. Failure to do so may result in your offer being withdrawn.
