Day Rate: £320 Ltd / £251.45 PAYE (excluding holiday pay)

Service Care Solutions are currently recruiting for an experienced Continuing Health Care (CHC) Nurse Assessor to join an NHS service working with adult patients in Northampton. This is a full?time role offering a competitive rate and the opportunity to work within a supportive multi?disciplinary team.

Key Responsibilities
Undertake Adult Continuing Health Care assessments in line with the National Framework
Complete DSTs, checklists, reviews, and reassessments
Facilitate and participate in multi disciplinary meetings
Work collaboratively with health, social care, and commissioning teams
Develop and review holistic, person?centred care plans
Maintain accurate and up?to?date clinical documentation
Support discharge planning and ongoing community care arrangements
Liaise with service users, families, and professionals to ensure effective care delivery Requirements
Registered Nurse (Adult Nurse preferred)
Previous experience working as a CHC Nurse Assessor with adults
Strong understanding of the Adult CHC National Framework
Experience in multi agency working
Ability to work independently and manage workload effectively
Excellent communication and organisational skills
