- Provide skilled nursing care to patients in their homes, helping them maintain independence and recover comfortably.
- Assess patient conditions, administer medications, perform wound care, and update care plans in collaboration with physicians and therapists.
- Active registered nurse license.
- At least one year of clinical experience in home health or a related field.
- Proficiency with electronic medical record (EMR) systems.
- Strong communication, patient education, and the ability to work independently.
- SSN and DOB required.
- Experience with EPIC.
