The Medical Group is a system-led operating entity formed to deliver high quality, low cost, innovative community-based ambulatory care, offering primary care, specialty care, behavioral and mental health, urgent care, outpatient surgery and endoscopy, imaging, cardiac testing, and infusion across locations in Massachusetts, New Hampshire, and Maine.

We are seeking a Full-time 40-hour Social Worker to support our office Monday through Friday from 8:30am-5:00pm. This hybrid position entails onsite coverage 3-4 days per week at our Family Medicine office located at 30 Locust Street, Northampton, MA, with remote coverage 1-2 days per week. Days can be flexible dependent on candidate schedule.

Due to the hybrid nature of the role, employees receive required technology and training to be proficient in all job responsibilities regardless of work location. Employees must designate a workspace that is private, safe, ergonomic, and free from distractions for all hours worked.

Job Summary

Our social worker will work within the Integrated Behavioral Health Service; collaborating with primary care and other specialty services to deliver exceptional care to patients across the age spectrum. There are opportunities for professional growth, including participation in improvement projects focused on efficiency and care delivery. We prototype new ideas, including digital tools, care team roles, and workflows that may be used in designing the future of ambulatory care across MGB Integrated Care.

Responsibilities
  • Providing short-term, evidence-based psychotherapy (individual, families and groups) to patients of the primary care practice in-person and virtually
  • Developing comprehensive assessments and treatment plans for patients engaged in behavioral health treatment
  • Providing a range of interventions to support primary care as needed such as crisis intervention; care coordination; information; referrals; and safety planning
  • Assessing new patients to aid in identifying the best treatment options
  • Participating in formal and informal case reviews, seminars and inter- and intra-departmental peer review meetings
  • Supporting patients in accessing resources and managing care
  • Working closely with other members of the Behavioral Health Team, Primary Care, and Collaborative Care Teams to coordinate services
Qualifications

Education
Master's Degree in a Related Field of Study required

Licenses and Credentials
Licensed Clinical Social Worker (LCSW) — State License;
Licensed Independent Clinical Social Worker (LICSW) — State License;
Licensed Marriage and Family Therapist (LMFT) — State License;
Licensed Mental Health Counselor (LMHC) — State License. HR Only preferred for all licenses.

Experience
Related experience 3-5 years required, with demonstrated ability to work with diverse backgrounds and lifestyles, engage in continual learning, and experience negotiating, advocating, and navigating systems.
