The Center for Human Development (CHD) is seeking a Case Manager to join our team at Grace House, a residential treatment program for mothers in early recovery. The program can house up to twelve mothers with children in a collective living environment, building a recovery community and encouraging peaceful living.
Your Role as a Case ManagerThe Case Manager provides support services to women with children and/or pregnant in a comprehensive six-to-nine‑month residential treatment program. The manager assists referred individuals in need of substance abuse treatment by providing the education and support needed to establish a clean and sober way of independent living and by helping clients access community resources and mitigate barriers to success. We strive to provide each family with a safe, nurturing environment where mothers and their children can experience success.
Schedule:
11:00am‑7:00pm, Tuesday‑Saturday.
- Provide service coordination and linkages to the community
- Develop helping relationships using programmatic structure and evidence‑based models including Motivational Interviewing (MI)
- Connect clients to varying community entities and help find resources for successful treatment and discharge
- Facilitate groups for up to 12 women at a time
- Assist clients in obtaining benefits, housing and health care as needed
- Provide support and participate in the development, revision, and utilization of relapse prevention and/or safety plans
- Encourage and facilitate utilization of clients’ natural support systems as well as recovery‑oriented, peer support and self‑help services
- Assist in data collection and tracking for grant evaluation
- Maintain and update appropriate treatment plans for residents
- Bachelor’s degree from an accredited institution or at least three years of relevant work or lived experience acceptable by MBHP waiver
- Ability to pass certification for CPR and First Aid
- Valid driver’s license and ability to provide transportation as needed
- Ability to counsel and support clients with co‑occurring and/or substance use disorder diagnoses
- Demonstrated knowledge of trauma‑informed care
- Bilingual in Spanish/English encouraged
The ideal Case Manager requires empathy and a high level of patience as minor and major setbacks often occur during the recovery process. Candidates with lived experience relative to co‑occurring and/or substance use disorder preferred. Recovery Coaches welcome.
